Clients are referred to the CHAMPS clinic by their Prisma Health pediatricians. Our team helps families with the following issues and case types:

• Children’s SSI Disability
• Children’s Medicaid
• Supplemental Nutrition Assistance Program (SNAP)
• Temporary Assistance to Needy Families (TANF)
• Housing Conditions
• Education (including special education, Section 504 matters, student discipline matters)
• End of Life Planning
