Clients are referred to the CHAMPS clinic by their Prisma Health pediatricians. Our team helps families with the following issues and case types:
- Children’s SSI Disability
- Children’s Medicaid
- Supplemental Nutrition Assistance Program (SNAP)
- Temporary Assistance to Needy Families (TANF)
- Housing Conditions
- Education (including special education, Section 504 matters, student discipline matters)
- End of Life Planning
